Continuing the baptismal promise in the Christian education of my child
(This information is for office use & to be shared with Learning Time teacher.)

In baptism you promised to bring your children to the services of God’s house, teach them the Lord’s Prayer, the Creed and the Ten Commandments . . . place in their hands the Holy Scriptures and provide for their instruction in the Christian faith.

I would like to have my child(ren) continue growing in faith. I give permission for my child(ren) to attend all education and youth functions of First English Lutheran Church and to receive emergency medical treatment as needed. I understand the staff will try to contact me as soon as possible in the event of an emergency.

Photo release: Pictures of youth engaged in First English-sponsored youth activities may be used on the First English Web site, the First English Family & Youth Web site, other FELC electronic media and in promotional materials, such as brochures and posters. Your child(ren) may be in one or more of these pictures. By signing below, the parent warrants and represents that the parent is the legal guardian of the child(ren) and has the full legal capacity to execute this release to give permission for pictures to be published by FELC.
